Why Join

Your patients will receive high quality care and enhanced benefits with one health plan responsible for coordinating their medical care, long-term care, mental health and substance abuse care, and social service needs. Coordinated care removes the uncertainty you and your patients may currently experience in trying to determine which agency – Medicare or Medi-Cal – to contact for care.

Easy to Use

One telephone number to call, one health insurance card, and one set of benefits.

Person-Centered Care

L.A. Care is focused on meeting each individual's needs, working with our members, their family members and other caregivers to improve health care and access to home- and community-based services.

Member Materials in Multiple Languages

Services and member materials are provided in the member's language.

Dignity and Independence

L.A. Care supports and empowers our members to actively manage their chronic conditions. The goal of the Cal MediConnect program is to help members avoid unnecessary hospitalizations and placement in long-term care facilities, and to lead more independent lives in the comfort and familiarity of their own homes and communities.

Member Services

Members will have access to member services representatives who speak their language via telephone for questions and assistance, 24-hours a day, 7 days a week, including holidays.

Access to More Resources

Through this program, members will have access to a free nurse advice line, free chronic disease management programs, free health education workshops, and a social worker, as needed.

Additional Services for NO Additional Cost

Dual eligibles will get all the coverage they have now with Medicare and Medi-Cal, but it will be combined into ONE PLAN. They will also get more personal help with the services they need when they need them—all at no extra cost.
